This is an update to the alert issued by the TGA on 16 June 2011. Johnson and Johnson have advised the TGA of additional serial numbers affected by the recall.

Additional devices have been identified as being affected by the 'Error 2' message.

This recall follows a recent increase in the number of failures of CI512 implant in the Cochlear Nucleus CI500 Implant range.

About Octagam 5%: lifting of the voluntary recall

TRIAD Sterile Lubricating Jelly, 3g sachet, with lot numbers beginning with 7, 8, 9 or 0, is being recalled as it may not have been properly sterilised.

Product may not have been properly sterilised.

Kits with the above item number and barcode number contain a thermometer which displays the temperature reading in degrees Fahrenheit rather than degrees Celsius.
